General information on Breast Augmentation
- Anesthesia: General or Neuroleptanesthesia (intoxication)
- Duration: 1-2 hours
- Stay: Usually 1 day
- Pain: Moderate pain when the implant is above the muscle, more severe pain when the implant is inserted under the muscle. Painkillers will be required during the first days
- Oedema - Swelling: Usually 10 days if the implant is above the muscle, and 20 days if it is below the muscle
- Bandages (Gauze): Removed in 1-3 days, Stitches: They are absorbable, no need to remove them
- Bruises: They improve by day 10. Some people do not get any bruises
- Hypaesthesia: Usually lasts 1-2 months, rarely up to 6 months
- Support: A sports bra must be worn for 3-4 weeks
- Drainage: Usually 1 day,
- Work: Return to work in 3-7 days. If the work requires weight lifting or a lot of movement, 2-4 weeks may be needed,
- Exercise: Return to the gym in 3-4 weeks
- Driving: As usual
- Final result: Usually 1-2 months for implants above the muscle; up to 4 months for implants below the muscle.
During your initial visit, the doctor will ask you to describe in detail what it is you would like to improve. The doctor will examine if there are any asymmetries, if there is ptosis, and together you will discuss the surgical plan.
You will need to discuss:
- The shape of the implant
- Its surface (textured surface)
- The implant volume (in cubic centimeters)
- The position where it will be inserted (nipple, inframammary, armpit etc.)
- The position of the incision (nipple, inframammary, armpit etc.)
You must state the following in your medical history:
- If you have problems with the heart, lungs, liver etc.
- If you suffer from hypertension
- If you have a family history of thrombophlebitis
- If you suffer from diabetes mellitus
- If you are smokers
- If you take medicine or food supplements (and which ones)
It is recommended that the surgery does not take place during menstruation

Breast Augmentation: The Operation
- The surgery usually lasts up to two hours; it takes place in an organized surgical center (Hospital) and it is usually performed with general anaesthesia, although in some cases (this is something that you will discuss with your doctor) it may be performed with local anaesthesia.
The operation will be preceded by laboratory testings (Electrocardiogram, chest X-ray, blood tests)
Breast augmentation aims to increase the size of the breasts. It does not improve asymmetries of the nipples and it does not help sagging breasts (in this case a breast lift will be necessary).
- Implant shape:
There are two types of implants: Round Breast Implants and Tear Drop Implants
- Recent studies have shown that round implants are superior to tear drop implants because:
- they are not affected by the possible movement of the implant
- they also have the shape of a drop in an upright position
- they appear more natural in a horizontal position
- they have a significantly lower re-operation rate
which is why the majority of plastic surgeons use round breast implants.
Implant Surface
- There are Textured Breast Implants and Smooth Breast Implants
- In Europe we traditionally use textured implants, as they do not easily move from their initial position and they have better texture (In the U.S.A. they use mainly smooth implants)
Implant Volumes
- It is not easy to decide on the volume of the implant
- Your doctor's experience and your consultation with them will assist you in your decision
- Some sizers could potentially help
- For implants larger than 400 ml there is usually higher chance that they might move from their initial position
- Do not hesitate to show your doctor pictures of breasts that you might like (you may not achieve the same result, as they are always differences that your doctor will point out to you, for example different anatomy, different breast diameter, sides or sternum going inwards or outwards etc.)
Implant Position
- It can be positioned above or below the pectoralis major muscle
- When located below the muscle (because it is transected) there is more postoperative pain and swelling
- In athletes it is usually positioned above the muscle
- In theory, when positioned below the muscle there is a lower rate of capsule formation, but a higher "damage" rate due to friction with the muscle.

Incision area: The incisions where the implant may be placed are:
- Periareolar (around the nipple)
- inframammary (inframammary crease)
- axillary
The decision will be made depending on your anatomical data (e.g. large nipple, small ptosis of the breast) and after consultation with your plastic surgeon.
Recovery
- on the first day you will experience breast pain, pressure, strain and discomfort in general
- the pain will be more severe if the implants are below the muscle (up to two weeks); painkillers will be necessary
- possible bruising up to 2 weeks
- oedema – swelling up to 3 weeks
- antibiotics for a week
- special sports bra for 1 month
- no sleeping on your stomach for 2 weeks
- no exercising, no heavy weight lifting, no sexual intercourse for 3 weeks
- swimming in the sea only after 3 weeks
- follow-ups are recommended every 6 months in the doctor's office
- depending on the age and family history, magnetic resonance imaging (MRI) is recommended every 2 years
- massaging is not necessary on textured breast implants (only on smooth breast implants)
- the results will be visible after approx. 2 months, when the breasts will be soft without swelling and pains.
Possible Complications
- Complications, although rare, must be reported and treated accordingly:
- reaction to anaesthesia
- asymmetry
- hematoma (<1% mainly during the first 24 hours; take care when taking food supplements)
- capsule (2% reaction of the tissue around the breast, which becomes harder. There is a classification according to hardness (Baker) in 4 categories
- hypaesthesia
- inflammation
- implant injury
- symmastia
- visible scar
- rippling
- skin necrosis
- galactorrhea
Extensive consultation with the doctor and understanding of instructions limits the chance of complications
Attention: For women with a family history of breast cancer, breast augmentation may be contraindicated
Attention: The laboratory testing of the breasts (U/S, MRI etc.) must be done on a regular basis
Breast Augmentation - Instructions
A few days before the operation
- Supplying of a special sports bra, which will be worn on the day following the operation
- Performance of the necessary preoperative check (examinations)
- Ensuring that you are not pregnant
- Scheduling of the operation so that it does not take place during the days of menstruation (period)
On the day of the operation
- Supplying of Arnica cream, antibiotics and painkillers (with prescription).
- Do not eat or drink anything if the operation is scheduled before noon, only a little coffee or tea with one rusk at least six hours before the operation if it is scheduled for the evening.
- Make sure to bring a nightgown, preferably with buttons, so that it can be easily worn after the surgery
- Avoid wearing jewelry
- Nails must be clean without polish.
After the operation
- During the first 24 hours, the elastic bandage surrounding and pressing the thorax might cause mild discomfort
- It might cause light swelling at the height of the clavicle. This will subdue after a few days
- Potential pain will be reduced in 2-3 days.
- Light diet with yogurt, jelly, soup, rusks etc. is recommended.
From the next day of the operation
- Removal of the elastic bandage, which brings instant relief.
- The sports bra must be worn for one month; however, it can be removed during showering
- During the first 2-3 days, an elastic bandage may be placed above the bra on the upper pole of the breast which is swollen so as to speed up the result
- Normal diet without restrictions.
General Instructions - Information
- The sports bra must be worn for at least a month.
- From day 15, the breasts should be massaged with circular movements using a moisturizing cream, in accordance with the provided instructions.
- Avoid lifting weights for 15 days after the operation. Begin exercising gradually after three weeks.
- Rare cases of complications (hematoma, inflammation, capsule) will be treated accordingly as discussed preoperatively.
- We always request the special guarantee paper of all implants used.
- Cases of small breast asymmetry may exist after the operation as well

CV- Dimitris Triantafyllou
Dimitris Triantafyllou graduated from the Experimental School of the University of Thessaloniki, as well as from the Military Medical School (SAAS) with honors.
He was further educated in the United States and more specifically in the Washington University of St. Louis, with which he maintains close relations and which he visits annually. He was director of Healthcare in the Dodecanese, curator in 424 General Military Training Hospital (GSNE).
He held the rank of Chief Medical Officer and the position of Plastic Surgery Director in 424 GSNE; he retired with the rank of General Chief Medical Officer (Colonel). He has presented scientific papers and speeches in many conferences. He is a member of the Plastic Surgery company (www.herpas.gr) and a candidate Member of the American Society of Plastic Surgeons. He has been a Research Fellow and a Keynote speaker for the GENESIS Hospital for 10 years, as well as a Research Fellow of Euromedica in Rhodes.

CV- Triantafyllos Vrizas
He was born in Drama on 1964 and has graduated from High School. He is a graduate of the Medical School of the Aristotle University of Thessaloniki (1982-1988).
He served in the army and the Community Hospital of Vathytopos (Nevrokopi, Prefecture of Drama) (1988-1990).
He completed the prerequisite training in Surgery and Orthopedics in the General Hospital of the Prefecture of Drama, as well as Plastic Surgery training in Great Britain (Whiston Hospital - Liverpool, George Eliot Hospital Birmingham, Devon and Exeter Hospital, Exeter) in a paid trainee position, grade of House Officer, Registrar and Senior Registrar (1992-1997).
He acquired his specialization title in March 1998 and he has been practicing medicine privately since 1998 in Thessaloniki. He is scientific director of the Plastic Surgery department of the GENESIS Hospital and he collaborates with the Hospitals of St. Loukas, Kyanous Stavros, Interbalkan Medical Center and General Hospital of Thessaloniki. He is working on a doctoral dissertation in A.U.TH., while he has been on post-training trips for information on new techniques in the U.S.A., Sweden, Netherlands and Great Britain.
